About Us
Founded in 2010, we are a mobile wound care provider serving NJ, PA, and IL, delivering care directly to long-term care and assisted living facilities using advanced technology and personalized approaches to improve patient outcomes.
Role Responsibilities
- Conduct assessments and develop personalized treatment plans
- Provide hands-on bedside wound care in skilled nursing and assisted living facilities
- Lead wound assessments, procedures, and treatment planning
- Utilize innovative tech and evidence-based protocols to promote healing
- Collaborate with facility teams and families for holistic care decisions
- Maintain accurate EMR documentation and support quality initiatives
Qualifications
- Active PA Nurse Practitioner license (board-eligible not accepted) or Physician Assistant
- Prescriptive authority and DEA license (must obtain within training period)
- Minimum 2 years wound care experience
- Experience in SNFs or long-term care preferred
- Willingness to travel within Cumberland/Franklin Counties
- Comfortable with EMR and mobile documentation
- Strong interpersonal and collaborative skills
