We are seeking compassionate Speech Language Pathologists (SLPs)/Speech Therapists (STs) for part-time, per diem home health visits across several counties, with potential for full-time transition.
Provide skilled speech therapy interventions, assessments, and case management in patients’ homes. Monitor patient progress, ensure compliance with physician orders, and educate patients and caregivers. Assess communication, oral-motor, cognitive, and hearing functions; develop and revise care plans accordingly. Coordinate care with other health professionals, report changes in patient status, and prepare patients/families for discharge. Maintain current knowledge through continued education and participate in team meetings. Qualifications include a Master’s degree in speech pathology, current licensure, ASHA certification, and at least one year of experience. Offer competitive pay ($100-$120/hour), mileage reimbursement, flexible schedule, and supportive team environment. Work involves traveling to patient homes, with schedule flexibility weekdays and weekends as needed.
