Seeking a Speech Language Pathologist (SLP) with CCCs (no CFY) for post-acute care, preferably with Skilled Nursing and Long Term Care experience.
Provide direct speech therapy rehab services based on physician referrals, adhering to all regulations and standards. Require a graduate degree (MS or MA) in speech therapy from an accredited program, with ASHA certification (CCCs) or eligibility for a 9-month CFY, maintaining credentialing throughout employment. Must hold an active state license, with at least one year of experience in post-acute settings preferred. CPR certification required upon hire or during orientation, with ongoing compliance. Participate in community health initiatives, demonstrate proficiency in Microsoft Office, and collaborate effectively with patients, families, and team members. Perform documentation accurately, utilize therapy software, and follow department policies and procedures. Establish and evaluate patient goals, oversee CFYs if applicable, and ensure safety, confidentiality, and professionalism at all times. Maintain dependable attendance, excellent communication skills, and physical ability to perform typical healthcare duties.
