This role provides in-home speech therapy services to patients in the San Jose area, focusing on speech, language, cognitive-communication, and swallowing disorders.

Conduct initial evaluations and develop personalized care plans

Deliver skilled therapy sessions, monitor progress, and document outcomes

Collaborate with interdisciplinary teams and educate patients and caregivers

Complete required documentation, including assessments like OASIS

Ideal candidates are licensed Speech-Language Pathologists with at least one year of experience, compassionate, organized, and capable of working independently while delivering high-quality, patient-centered care.
