Seeking Board Certified Geriatricians, Internists, and Family Practitioners to join a dedicated team providing comprehensive on-site medical care in post-acute, rehabilitation, long-term care, continuing care communities, and assisted living centers.
Deliver face-to-face primary care services across a spectrum of post-acute facilities, from short-term rehab to complex geriatric patients in skilled nursing settings. Participate in monthly educational lectures and clinical outcomes reviews to enhance clinical skills and knowledge. Collaborate with Nurse Practitioners and Physician Assistants to ensure coordinated patient care.
