Seeking Board-Certified or Board-Eligible Family Medicine/Internal Medicine Physicians for immediate full-time roles in Fort Collins, Greeley, Loveland, and Windsor, CO.

  • Provide compassionate care across post-acute and long-term care settings, including skilled nursing, assisted living, and independent living.
  • Collaborate with interdisciplinary teams to ensure quality, safety, and regulatory compliance.
  • Create timely documentation, support care coordination, and participate in quality improvement activities.
  • Travel between multiple facilities as needed; reliable transportation required.
  • Share minimal rotating weekend and holiday call (no night call).

    **Qualifications:**
    Required: MD or DO, Colorado medical license or eligibility, strong communication skills, ability to work independently and in a team.
    Preferred: Experience in post-acute care or hospitalist settings; familiarity with relevant software.
