Responsibilities include conducting assessments, managing acute and chronic conditions, ordering diagnostics, prescribing medications, and educating patients and staff. The role offers autonomy, flexible scheduling, minimal on-call duties, and a supportive team environment.
Qualifications: Current MN NP license, board certification, and at least 1 year of experience in a similar setting preferred. Strong geriatric and primary care skills, excellent communication, and a commitment to patient-centered care are essential.
