Seeking a passionate Primary Care/Mental Health Integration LCSW for a Community Outpatient Clinic in Bowling Green, KY.
Provide brief, solution-focused interventions for veterans, including assessments, individual therapy, and crisis management.
Engage veterans and team members in treatment planning, addressing behavioral and physical health issues like depression, anxiety, diabetes, and heart conditions.
Perform emergency care, coordinate hospital admissions, and serve as a liaison with families.
Conduct rapid psychiatric assessments, crisis intervention, and case management throughout the care continuum.
Input data into EMR, maintain confidentiality, and participate in program development, staff meetings, and performance improvement.
Ensure compliance with VA standards, foster excellent customer service, and support outreach and enrollment initiatives.
Qualifications include a Master’s in Social Work or equivalent, valid unrestricted license, 1-3 years of adult counseling experience, and proficiency in EMR and English.
Must pass PCMHI certification and maintain BLS certification.
