Join our team as an Advanced Nurse Practitioner to oversee hospice patient care, collaborate with physicians, and ensure compliance with regulations. Responsibilities include developing and implementing the Plan of Care, managing symptoms, reviewing diagnostics, maintaining documentation, and providing primary and urgent care. Travel may be required.
Requirements include:
- Master’s Degree in Nursing and current licensure
- Active DEA license
- At least 1 year of hospice or home care experience
- Knowledge of hospice and palliative care principles
- Preferred certification in Hospice and Palliative Care
- Strong collaboration and communication skills within interdisciplinary teams
