Seeking a Speech Language Pathologist for a home care role providing personalized evaluation and treatment to improve communication, cognition, and swallowing in patients’ homes.
Conduct thorough assessments using standardized tools to evaluate speech, language, voice, fluency, and swallowing abilities.
Develop and adjust individualized, evidence-based treatment plans aligned with patient goals and home environment considerations.
Document assessments, interventions, and progress in compliance with policies and regulations, ensuring confidentiality and proper reimbursement.
Educate patients and caregivers on communication strategies, safe swallowing, and compensatory techniques within daily routines.
Collaborate with interdisciplinary teams, participate in care planning, and recommend assistive technology or equipment when needed.
Maintain licensure through ongoing education and stay current with clinical best practices.
Mentor peers and contribute to team development as needed.
Qualifications include a Master’s in Speech-Language Pathology, CCC from ASHA or CFY, current NY State license, and CPR certification. Experience with dysphagia and neurogenic disorders is preferred.
