Join a dedicated team as an Advanced Practice Provider (PA or NP) in Family Medicine in Prineville, Oregon within a Medical Home Model.
Provide comprehensive, patient-centered care to patients of all ages, collaborating with behavioral health, pharmacy, care managers, and specialists. Work full-time (40 hours/week) with a minimum of 36 patient contact hours and participate in a clinic call rotation, including evenings, weekends, and holidays. Engage in evidence-based family medicine with a focus on prevention, ensuring EHR documentation, coding, and billing compliance. Maintain ongoing education and certifications. Requirements include a Master’s degree (Doctorate preferred), unrestricted Oregon PA/NP license, national certification, DEA & BLS certifications, and preferably 2+ years of outpatient family medicine experience. Experience with EHR systems (Epic preferred) and familiarity with the Medical Home Care Model are advantageous.
