The Case Management Coordinator collaborates with patients, families, physicians, and the interdisciplinary team to ensure efficient and cost-effective patient care progression.
Responsibilities include managing patient flow, facilitating safe transitions, discharge planning, and supporting continuity of care to enhance patient satisfaction and safety.
Provides comprehensive assessments, care coordination, and evaluates individual patient needs while adhering to organizational policies and standards.
Handles payer requests related to individual members when appropriate.
Requires a graduate of an accredited nursing program (BSN preferred) or a Master’s in Social Work (MSW) and valid licensure as a Registered Nurse or LMSW/LISW/LCSW in New Mexico.
Case management certification (ACM, CCM, RN-BC) is highly desirable.
Must demonstrate effective communication, teamwork, critical thinking, multitasking, and organizational skills, with knowledge of community and post-acute resources.
Preferred candidates have at least two years of healthcare experience, including one year in an acute care setting.
Reports to the Director of Case Management.
Works primarily in a hospital and office environment, with mobility around hospital units.
Physical requirements include mobility, good vision, dexterity, and the ability to work in a fast-paced environment.
