Seeking an Associate Marriage and Family Therapist (AMFT) or Associate Clinical Social Worker (ACSW) to join a large community health system’s mental health team.
Provide counseling to patients, complete accurate documentation, and develop individualized care plans.
Address co-occurring disorders and collaborate within a multidisciplinary team.
Coordinate with case managers and connect patients to internal/external resources.
Must have active registration in good standing with the State of California as an AMFT, APCC, or ACSW.
Work in outpatient settings supporting individuals with mild to moderate mental health needs.
