We are seeking Board Certified Geriatricians, Internists, and Family Practitioners to join a dedicated team providing comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living communities.
Provide face-to-face primary care services across various post-acute settings, from short-term rehab to complex geriatric patients in skilled nursing facilities.
Participate in monthly educational sessions and clinical outcomes reviews.
Collaborate with Nurse Practitioners and Physician Assistants to enhance patient care.
