We are seeking a qualified MD/DO for a contractor role focused on establishing patient relationships for chronic care management services.
Responsibilities include conducting approximately 375 clinical visits per month, both onsite and remotely, with travel up to 1-2 hours from home, compensated by mileage.
Accurately document patient encounters in the platform and provide end-of-day reports to program administrators.
Availability of at least 12 working days per month is required.
Minimum requirements include active, unrestricted medical licenses in Illinois and at least 10 other states, reliable transportation, and familiarity with EHR systems.
Must demonstrate excellent clinical judgment, patient-centered care commitment, and ability to travel within designated regions.
The position offers a compensation range of $7,000 to $10,000 per month, depending on experience and location.
