We are seeking Board Certified Geriatricians, Internists, and Family Practitioners to join a dedicated team providing comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living communities.
Provide face-to-face primary care services to a diverse patient population, including short-term rehabilitation and complex geriatric patients.
Participate in monthly educational lectures and clinical outcomes reviews to foster continuous learning and improvement.
Collaborate with Nurse Practitioners and Physician Assistants to ensure coordinated patient care.
