We are seeking a compassionate, full-time Clinical Social Worker to provide psychosocial care to patients and families in northern Anne Arundel County.
Offer psycho-social assessments and develop individualized care plans addressing emotional, spiritual, environmental, and financial needs.
Collaborate with multidisciplinary teams, including RNs, chaplains, CNAs, and physicians, to ensure holistic, patient-centered care.
Participate actively in team meetings, advocate for patients, and communicate care plans effectively.
Provide counseling, emotional support, crisis intervention, and facilitate referrals to community resources.
Maintain thorough documentation in electronic medical records and follow up on care outcomes.
Qualifications include a Master of Social Work, LMSW license (LCSW-C preferred), 2-3 years healthcare experience, and a valid driver’s license.
The role emphasizes care continuity, quality, and empowering social workers to manage their schedules.
Candidates must be vaccinated against COVID and influenza, with TB screening upon hire and annually.
