Seeking a passionate Primary Care/Mental Health Integration LCSW to join a team at a Community Based Outpatient Clinic in Cookeville, TN.
Provide brief, solution-focused interventions, assessments, and treatment planning for veterans with physical and behavioral health issues.
Conduct evaluations, engage veterans and team members, and deliver individual therapy for conditions like depression, anxiety, diabetes, and heart conditions.
Offer crisis intervention, emergency care, case management, and coordinate inpatient or psychiatric referrals.
Advocate for veterans, educate on self-advocacy, and address psychosocial needs impacting health and treatment adherence.
Maintain documentation in EMR, participate in staff meetings, program development, and outreach activities.
Ensure compliance with VA standards, participate in performance improvement, and support strategic initiatives.
Qualifications include a Master’s in Social Work (or equivalent), valid LCSW license, 1-3 years’ experience with adult patients, and proficiency in EMR and English.
