We are seeking an Advanced Nurse Practitioner to oversee and coordinate care for hospice patients, collaborate with physicians, and ensure compliance with regulations.
Major responsibilities include implementing and reevaluating the Plan of Care, managing medication adjustments, completing face-to-face evaluations, and serving as the primary on-call clinician for urgent needs.
Requirements:
– Master’s Degree in Nursing
– Current state licensure and DEA license
– 1+ year home care or hospice experience
– Knowledge of hospice and palliative care principles
– Preferred certification in Hospice and Palliative Care
