Responsibilities include street outreach, coordinating needs pre- and post-housing, enhancing daily living skills, offering supportive counseling, and advocating against discrimination and healthcare disparities.
The role emphasizes member choice, harm reduction, non-coercion, flexibility, and person-centered care. Post-housing, the team supports participants for stability and independence, with fieldwork, on-call duties, and flexible hours required.
