Field Nurse Practitioner

We are seeking dedicated Nurse Practitioners to provide in-home care for high-risk, high-need patients enrolled in AHCCCS and Medicaid plans.

Assist patients by examining, reviewing records, creating treatment plans, prescribing medications, and educating families.

Coordinate with healthcare teams to ensure seamless care and address barriers within patients’ homes.

Manage a caseload of approximately 100 members, visiting 5 patients daily, and work closely with support staff including medical assistants, social workers, and behavioral health specialists.

Focus on improving medical outcomes over a one-year period and supporting patient goals for better health and quality of life.

Qualifications include active Nurse Practitioner licensure in Arizona, strong communication skills, and experience in primary, acute, or chronic care. Bilingual in Spanish is preferred.

Compensation starts at $130,000 annually, with benefits such as insurance, retirement plans, paid time off, and performance bonuses. This full-time role covers the Yavapai East area.
