Annual salary of $135,000–$140,000 with bonus potential
$15,000 sign-on bonus

Role Overview:
Conduct assessments and develop personalized wound care treatment plans
Deliver bedside wound care and perform advanced procedures in long-term and assisted living facilities
Utilize innovative technology and evidence-based protocols to support healing
Collaborate with facility teams and families, document in electronic records, and support quality initiatives
Qualifications:
Active PA Nurse Practitioner license or Physician Assistant license
Prescriptive and DEA licenses (must obtain within training)
Minimum 2 years of wound care experience and prior work in SNFs or long-term care preferred
Willing to travel within designated areas and comfortable with EMR documentation
