Field Nurse Practitioner

Nurse Practitioners in this role provide in-home care to high-risk, high-need Medicaid and AHCCCS members, focusing on improving health outcomes through care coordination, education, and management of complex medical needs. They collaborate with a multidisciplinary team, including primary care providers, specialists, and social workers, to develop and implement personalized care plans, review diagnostic tests, prescribe and reconcile medications, and address barriers to care. The role involves engaging members around their goals, overseeing transitional care, and ensuring timely follow-up. Practitioners typically see five patients daily, managing a caseload of 100 members over a year. The position covers the Yavapai East area with a starting salary of $130,000, comprehensive benefits, and performance bonuses. Candidates must hold an Arizona Nurse Practitioner license, be proficient in primary and chronic care, and possess strong communication skills. Bilingual in Spanish is preferred.
